Please note that Performance Measures for the first quarter of Fiscal Year 2021 are due by close of business on Monday, Dec. 14, 2020.
Two forms are required: Beginning of the Fiscal Year and Quarter 1. Beginning of the Fiscal Year counts are as of Sept. 1, 2020. Quarter 1 includes ONLY cases opened during the quarter itself (Sept. 1, 2020 – Nov. 30, 2020).

- Remember that children served and race/ethnicity must be entered by county.
- When the forms are complete, use the button at the top of the page to Submit. The ODM will not allow submission of counts that do not add up. Data cannot be changed after clicking Submit.
